A Health Risk Assessment

A Health Risk Assessment is a self-reporting tool created to give clients and their employees an overall picture of individual health risks. The Health Assessment will provide an employee with a health risk report and offer suggestions on ways to reduce or eliminate those risks with healthy lifestyle choices.
